The Field Ambulance Units can date their origins back to 21 October 1899 when the Volunteer Medical Staff Corps was established in King William's Town in the present day Eastern Cape, then known as the Cape Colony. In 1974, it was re-formed from 23 Field Dressing Station as a Field Amulance Unit to support one of the newly organized SA Army formations. 20 Field Ambulance Unit was eventually combined with 3 Field Ambulance Unit and 12 Field Ambulance Unit to form 3 Medical Battalion Group on the Battalion Group's establishment on 5 November 1981.
