During Operation Sceptic, after capturing an ainti-aircraft position, Ratel 21C drew fire from a 23mm gun upon which the vehicle turned towards the position and continued towards it. The anti-aircraft gun started firing at the vehicle with intend and hit it several times. Rifleman Lello was in the process of debussing from the vehicle when presumable a hand grenade exploded in the vehicle. One of the members that were hit either had the grenade in his hand, or the grenade was detonated by a shot striking the vehicle. Shrapnel struck Rifleman Lello in the chest and face.
